**TS Badminton Waiver**

 I(We) hereby give permission for the staff of TS Badminton LLC, Terminus Sports LLC, Terminus Badminton, or TS Badminton Academy to seek appropriate medical attention for the athlete and for the medical attention to be given and for the athlete to receive medical attention in the event of accident, injury or illness. I will be responsible for any and all costs of medical attention and treatment.

I/We, the undersigned for ourselves, our heirs, executors and administrators waive, release and forever discharge TS Badminton LLC, Terminus Sports LLC, Terminus Badminton, or TS Badminton Academy, its staff, officers, directors, board members, coaches, agents, employees, representatives and successors and assigns of and from all rights and claims for damages, injury or loss to person or property which may be sustained or occur during participation in all athletic activities (including practices and tournaments/travel) — whether damages, injury or loss are due to negligence. I/We hereby acknowledge that I am / We are physically fit and mentally capable of participating in badminton activities.

 **TS Badminton Waiver (For Juniors)**

I/We the undersigned hereby certify that I(we) am(are) the parent(s) or legal guardian(s) of the athletes listed above. I(We) hereby give permission for the staff of TS Badminton LLC, Terminus Sports LLC, Terminus Badminton, or TS Badminton Academy to seek appropriate medical attention for the athlete and for the medical attention to be given and for the athlete to receive medical attention in the event of accident, injury or illness. I will be responsible for any and all costs of medical attention and treatment. I/We, the undersigned for ourselves, our heirs, executors and administrators waive, release and forever discharge TS Badminton LLC, Terminus Sports LLC, Terminus Badminton, or TS Badminton Academy, its staff, officers, directors, board members, coaches, agents, employees, representatives and successors and assigns of and from all rights and claims for damages, injury or loss to person or property which may be sustained or occur during participation in all athletic activities (including practices and tournaments/travel) — whether damages, injury or loss are due to negligence.

 **Photo Release**

I grant permission to TS Badminton LLC, Terminus Sports LLC, Terminus Badminton, or TS Badminton Academy the right to use photographs taken of my child participating in Terminus Badminton or TS Academy Programs for promotional use on its website at terminusbadminton.com and tsbadminton.com, and/or Facebook page, Instagram or Twitter account. I hereby agree to release, defend, and hold harmless TS Badminton LLC, Terminus Sports LLC, Terminus Badminton, or TS Badminton Academy, its staff, officers, directors, board members, coaches, agents, employees, representatives and successors from and against any claims, damages or liability arising from or related to the use of the photographs for promotional purposes.

 **Code of Conduct**

 - TS Badminton maintains an environment of respect and compassion in all of our events/clinics/lessons/sessions.
 - All students, members, and visitors that are utilizing our facilities must adhere to all rules and policies and are expected to act in a safe, courteous, and respectful manner at all times.
 - All requests and instructions by TS Badminton staff should be strictly followed and failure to do so may result in any person, including members, being directed to leave the premises.
 - Since group sessions will be scheduled at the same time as TS Badminton Academy coaching sessions, please show respect to students and mind your language during play.

Prohibited conduct at TS Badminton during on or off-premises includes, but is not limited to, the following:
 - Foul, profane, or abusive language toward another person.
 - Theft of any property.
 - Damaging the facilities of TS Badminton.
 - Threatening, harassing or intimidating any staff person, member, student, or visitor.
 - Use of illegal drugs.
 - Possession of guns, knives, or other weapons. - Smoking.
 - Providing false information or records to TS Badminton.
 - Selling products or conducting business without consent of TS Badminton. Any commercial activities inside the facility are strictly prohibited, including the sale of any badminton-related products or services. Violators will be immediately banned from entering the facility.
 - Additionally, any criminal activity or acts or threats of violence may be reported to the proper authorities and prosecuted. TS Badminton reserves the right to deny, suspend, revoke or limit access to events or facilities in the case of any violation of this code of conduct or any rule or policy, consistent with applicable law. 

**Indoor Court Shoe Requirement**

For the safety of all participants and to protect the facility flooring, all members, guests, and participants are required to wear proper indoor, non-marking court shoes while on the badminton courts. Shoes that leave marks, are intended for outdoor use, or may damage the court surface are strictly prohibited. TS Badminton Academy reserves the right to deny court access or participation to any individual not wearing appropriate footwear. No refunds or credits will be issued for denied participation due to non-compliance with this requirement.

**No Outside Coaching Classes Allowed**

All coaching and training activities at TS Badminton are reserved exclusively for authorized TS Badminton coaching staff. Outside coaches, trainers, or instructors are not permitted to coach, train, or provide instruction at TS Badminton under any circumstances. Players may not bring, invite, engage, or arrange for outside coaches to conduct private or group coaching sessions on TS Badminton premises. TS Badminton facilities may not be used by any external coaching personnel for coaching, training, instruction, player development, evaluations, or any similar activities, whether paid, unpaid, voluntary, complimentary, or otherwise.

Any questions arising regarding the meaning of any of the above rules should be brought to the attention of either the Head Coach (Thomas Shek) of TS Badminton or any of the owners of TS Badminton. The interpretation of any provision of this code/rule shall be final.
