2. Parent / Legal Guardian Information (if student is under 18)
**Code of Conduct**
- The Player agrees to always conduct himself/herself in a professional and sportsmanlike manner, including but not limited to, during tournaments, training sessions, public appearances, and social media interactions.
- The Player agrees to refrain from any behavior that could damage the reputation of the Player, TS Badminton and affiliated sponsors, or the sport of badminton.
- The Player agrees to treat other players, coaches, officials, and fans with respect.
- The Player agrees to comply with all applicable tournament rules and regulations.
- TS Badminton maintains an environment of respect and compassion in all of our events/clinics/lessons/sessions.
- All students, members, and visitors that are utilizing our facilities must adhere to all rules and policies and are expected to act in a safe, courteous, and respectful manner at all times.
- All requests and instructions by TS Badminton or Terminus Badminton staff should be strictly followed and failure to do so may result in any person, including members, being directed to leave the premises.
Prohibited conduct at TS Badminton during on or off-premises includes, but is not limited to, the following:
- Foul, profane, or abusive language toward another person.
- Theft of any property.
- Damaging the facilities of TS Badminton in any way.
- Damaging the properties of TS Badminton in any way.
- Threatening, harassing or intimidating any staff person, member, student, or visitor.
- Use of illegal drugs.
- Possession of guns, knives, or other weapons.
- Smoking.
- Providing false information or records to TS Badminton, Terminus Sports.
- Selling products without consent of TS Badminton.
- Additionally, any criminal activity or acts or threats of violence may be reported to the proper authorities and prosecuted.
TS Badminton and Terminus Badminton reserve the right to deny, suspend, revoke or limit access to events or facilities at its sole discretion in the case of any violation of this code of conduct or any rule or policy, consistent with applicable law.
**Liability Waiver**
I, the undersigned participant (or parent/guardian of a minor participant), understand and acknowledge that participation in sports activities at TS Badminton involves inherent risks, including but not limited to physical injury, illness, or property damage. I voluntarily assume all such risks associated with my/my child's participation.
In consideration of being allowed to participate in activities at TS Badminton, I hereby release, waive, and discharge TS Badminton LLC, TS Badminton Academy, Terminus Sports LLC or Terminus Badminton, its coaches, staff, directors, affiliates, and representatives from any and all liability, claims, demands, or causes of action arising out of or related to any injury, illness, or loss incurred while participating in academy activities, whether caused by negligence or otherwise.
I agree to follow all safety guidelines, wear appropriate gear, and adhere to the instructions of academy coaches and staff. I understand that failure to comply may result in removal from the training program.
I agree to follow all rules, regulations, and instructions set forth by TS Badminton. Failure to comply may result in suspension or dismissal from the academy without refund.
I grant permission for TS Badminton staff to seek emergency medical treatment for me/my child if necessary. I understand that I am financially responsible for any medical expenses incurred.
*For Adult Students*
I(We) hereby give permission for the staff of TS Badminton LLC, TS Badminton Academy, Terminus Sports LLC or Terminus Badminton to provide training to me. I/We hereby acknowledge that I am physically fit and mentally capable of participating in badminton activities.
*For 18 or Below Students*
I/We the undersigned hereby certify that I(we) am(are) the parent(s) or legal guardian(s) of the athletes listed above. I(We) hereby give permission for the staff of TS Badminton LLC, TS Badminton Academy, Terminus Sports LLC or Terminus Badminton to provide training to our child. I/We hereby acknowledge that our child is physically fit and mentally capable of participating in badminton activities.
**Marketing Communication Consent and Authorization**
By signing below, I hereby authorize TS Badminton to contact me using the information I have provided, including but not limited to my phone number, email address, and mailing address.
I consent to receive communications from TS Badminton for marketing, promotional, and informational purposes, which may include emails, phone calls, text messages (including automated or prerecorded messages), and other forms of communication. I understand that message and data rates may apply for text messages, depending on my mobile carrier.
I acknowledge that my consent is voluntary and that I may opt out of receiving marketing communications at any time by following the unsubscribe instructions provided in such communications or by contacting TS Badminton directly.
By signing below, I confirm that I am the authorized user of the contact information provided and that I agree to the terms outlined above.
Any questions arising regarding the meaning of any of the above rules should be brought to the attention of either the Head Coach (Thomas Shek) of TS Badminton or any of the owners of TS Badminton. The interpretation of any provision of this code/rule shall be final.
Indoor Court Shoe RequirementFor the safety of all participants and to protect the facility flooring, all members, guests, and participants are required to wear proper indoor, non-marking court shoes while on the badminton courts. Shoes that leave marks, are intended for outdoor use, or may damage the court surface are strictly prohibited. TS Badminton Academy reserves the right to deny court access or participation to any individual not wearing appropriate footwear. No refunds or credits will be issued for denied participation due to non-compliance with this requirement.
